Registration
- Each delegate must register online by 4pm on 17th April 2009 via the following link: REGISTER or by filling out a registration form (available here) and sending it to "Ear Health Conference, Locked Bag 3, Kalgoorlie WA 6433".

- Late registrations will be processed on site and will incur a A$50 late registration fee.
- Accommodation must be arranged by delegates directly with their chosen facility. See the “Accommodation” section of this website for more details

Earlybird - due 27/02/09 $300
Full registration - due 17/04/09 $350
Daily rates $200
Student rate $250
Payment
- Payment can be made in Australian dollars by cheque or credit card (Bankcard, Visa or MasterCard).
- Cheques are to be made payable to WACHS, Kalgoorlie Hospital. With accompanying documentation you must state that it is for the Ear Health Conference, Project ID 0006. Cheques to be posted to "Ear Health Conference, Locked Bag 3, Kalgoorlie WA 6433.
- Payment must accompany all registration applications. Reservations will not be confirmed until payment is received
- An invoice will be provided with your confirmation letter
Earlybird Rate
- To qualify for the Early Bird rate your registration and accompanying payment must be received by 4pm on 27th February 2009.
- If payment is not received by that date, the standard rate will apply.
Student and Research Assistant Rate
To be eligible for the reduced student and research assistant rate all the following conditions need to be met:
- Students must be in full-time study (please attach a photocopy of your Student ID with your registration form)
- Registration must not be otherwise funded
- Supervisors must sign the appropriate section on the registration form
Cancellations and Amendments
Registration cancellations and amendments as described below, must be notified in writing by email to goldfieldsearhealth@health.wa.gov.au
- Cancellations received before 31st March, 2009 will receive a refund less A$150.00 administration fee.
- As an alternative to cancellation, your registration may be transferred to another person without incurring any penalty, after arranging this with the conference organiser.
Cancellations or changes in accommodation arrangements are your own responsibility according to the terms and conditions of the accommodation venue you have booked.
